TMA Internship Forms

Overview of Academic Internships This general overview explains the responsibilities of everyone involved with your academic internship experience.
Student Responsibilities Checklist This checklist designed to help you comply with the requirements for obtaining credit for your internship.
Internship Application This application is specific to the Theatre and Media Arts Department and must be completed, signed (by you and your faculty advisor), and returned to the internship coordinator in order to be considered for an internship through the department.
Internship Objectives You must list at least 3 objectives you hope to achieve though your internship experience along with how you plan to meet those objectives.
Student Agreement Before you can register for your academic internship, you must read, sign, and return the Student Agreement stating that you will abide by the university's guidelines.
Internship Assignments This document explains the assignments you will be required to complete during and after your internship experience.
Internship Journal This form is to be used to submit your weekly journal entries.
Student Internship Evaluation This evaluation form is to be filled out by you at the completion of your internship experience.
Experience Provider/Internship Supervisor Evaluation of Student This evaluation form is to be filled out by your internship supervisor at the completion of your internship.
